PATIENT SUPPORT MOBILE TERMS AND CONDITIONS

QULIPTA (atogepant) Patient Support Program requires Consumers to enter their mobile number as a unique identifier upon registration in order to enable Two-Factor Authentication.


If Consumers consent, you will also receive Qulipta Patient Support Program updates and promotional offers via text message to their mobile device. Message frequency varies and message and data rates may apply. Consumers can unsubscribe from these messages at any time by texting "STOP". Assistance can be received by texting "HELP". If you unsubscribe, you are still able to receive transactional text messages upon request; ie, Forgot Password.


There is no fee charged by Qulipta Patient Support Program to receive messages. Message and data rates may apply–this can be confirmed with your mobile service provider. Charges are billed and payable to your mobile service provider or deducted from your prepaid account. Consent is not a requirement for purchase.


Data obtained from you in connection with this SMS service may include your mobile phone number, your carrier's name and the date, time, and content of your messages. We may use this information to contact you and to provide the services you request from us and this information will be used in accordance with our Privacy Notice and Terms of Use. Alerts sent via SMS may not be delivered if the mobile phone is not in range of a transmission site, or if sufficient network capacity is not available at a particular time. Even within a coverage area, factors beyond the control of the wireless carrier may interfere with message delivery, including the customer's equipment, terrain, proximity to buildings, foliage, and weather. The wireless carrier does not guarantee that alerts will be delivered and will not be held liable for delayed or undelivered messages.


We will not be liable for any delays in the receipt of any SMS messages as delivery is subject to effective transmission from your network operator. T-Mobile is not liable for delayed or undelivered messages.

IMPORTANT SAFETY INFORMATION

Do not take QULIPTA if you have had an allergic reaction to atogepant or any ingredients in QULIPTA.


Before taking QULIPTA, tell your healthcare provider about all your medical conditions, including if you:

  • Have high blood pressure
  • Have circulation problems in your fingers and toes
  • Have kidney problems or are on dialysis
  • Have liver problems
  • Are pregnant or plan to become pregnant
  • Are breastfeeding or plan to breastfeed

Tell your healthcare provider about all the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements. QULIPTA may affect the way other medicines work, and other medicines may affect how QULIPTA works. Your healthcare provider may need to change the dose of QULIPTA when taken with certain other medicines.


QULIPTA can cause serious side effects, including:

  • Allergic (hypersensitivity) reactions, including anaphylaxis: Serious allergic reactions can happen when you take QULIPTA or days after. Stop taking QULIPTA and get emergency medical help right away if you get any of the following symptoms, which may be part of a serious allergic reaction: swelling of the face, lips, or tongue; itching; trouble breathing; hives; or rash.
  • High blood pressure: New or worsening of high blood pressure can happen. Contact your healthcare provider if you have an increase in blood pressure.
  • Raynaud’s phenomenon: A type of circulation problem can worsen or happen. Raynaud’s phenomenon can lead to your fingers or toes feeling numb, cool, or painful, or changing color from pale, to blue, to red. Contact your healthcare provider if these symptoms occur.

The most common side effects of QULIPTA are nausea, constipation, and fatigue/sleepiness. These are not all the possible side effects of QULIPTA.


QULIPTA is available in 10 mg, 30 mg, and 60 mg tablets.
